Avers : En premier plan un ange déposant une couronne de lauriers sur la tête d'un sapeur-pompier tandis qu'en arrière-plan figure les situations dangereuses dans lesquelles le pompier peut être amené à intervenir. Dans le haut l'inscription « DÉVOUEMENT COURAGE ».
Revers : Branche de chêne et branche de laurier avec un cartouche vierge de toute gravure (absence du nom du récipiendaire) car la médaille n'a pas été décernée.
