Plaque en laiton estampé de forme carrée, 6,7 cm x 6,7 cm, avec bombe enflammée posée sur deux haches croisées, et branches de laurier et de chêne. En partie basse, un ruban porte l'inscription « DÉVOUEMENT » et un ruban en partie haute marqué « SAPEURS POMPIERS ».
